The national center requires sites that collect ASQ questionnaires via ASQ Online to submit ASQ CSV files annually by January 31st. The submission process is instead of these sites performing double data entry in both ASQ Online and DAISY. Please follow these steps:
Step 1. Identify which families/ASQ to include in the file
Use this table to determine which questionnaires should be included in the export file.

Please do not share questionnaires from families outside of the ParentChild+ program or questionnaires collected before the family started the ParentChild+ program.
Optional: Coordinators have the option to request a list of families that are in this cohort for reference. Please submit this request to support@parentchildplus.zendesk.com by January 17th.
Step 2. Download the reports from ASQ Online
- On the home screen, select Reports.
- Select the report you would like to download. Sites collecting ASQ-3 questionnaires should download the following reports: ASQ-3 Screening Scores and ASQ-3 Follow Up. Sites collecting ASQ:SE-2 questionnaires should download the following reports: ASQ:SE-2 Screening Scores and ASQ:SE/ASQ:SE-2 Follow Up.
- You have two options for filters before generating the report:
- Leave the filter options set to ALL or blank. This will give you a CSV file with every questionnaire stored on ASQ Online. In the CSV file, you will have to delete families who did not receive a visit in the calendar year and families who are not participating in ParentChild+ (if applicable).
- Use the screening date range to filter families who received a visit in the calendar year. This prevents you from having to manually delete families who did not receive a visit in the calendar year, but you will still have to delete families who are not participating in ParentChild+ (if applicable). Note: we are looking for all questionnaires, regardless of when they were screened, for families who received a visit in the calendar year.

- Example: if families currently receiving visits in 2024 started the program in 2022, enter "2022-01-01" to "2024-12-31" in the screening date range.

- Once you have decided what your filters should be, click Generate Report. When the file is ready, click Download CSV. Please do not share PDF files of the report.
Reminders:
- It is optional to de-identify children by omitting names and date of birth, depending on your agency’s policy. If you do so, please add the DAISY Child ID in the Alt. ID column. Alternatively, you can add the DAISY Child ID in the child profile in ASQ Online directly. See this article for details: How do I add the DAISY Child ID to child profile on ASQ Online?
- If you are not providing the DAISY Child ID, the file must include the child's first and last name, and date of birth.
- There is no need to include other columns in the file. However, if you are editing the CSV file before sharing, please remember the file must include the following: month interval of screener, the date the screener was completed, the value scores of each domain (ASQ-3) or total score (ASQ:SE-2) and follow up actions taken.
- ASQ Online has a few resources available for users, including this Report Guide. I also found this video on YouTube that discusses using ASQ Online. As you can see around 19:17, they show viewers where the reporting section is (i.e., clicking Reports on the navigation bar) and the full list of reports (which is where they will find the ASQ-3 Screening Scores, ASQ:SE-2 Screening Scores ASQ-3 Follow Up, ASQ:SE-2 Follow Up reports we asked for). Step 3. Share files
All data submissions are due on January 31st. They should be uploaded to the Annual Requirement section in DAISY. In the pop-up window, select the type of data export you are submitting in the Document Type dropdown.
