The ASQ-3 is a questionnaire collected pre-program, mid-program, and post-program for each in the 1:1 model. To add an ASQ-3, follow these steps:

Click ADD FORM.

Select ASQ-3 from the pop-up window.
- Type in the administration date (the date which these data were collected)

Select who completed the ASQ-3. This dropdown will include all adults listed in the Family Information form.

Select the type (pre, mid, post or additional)

Select the month interval for the ASQ-3 questionnaire you are adding.

Select the exemption status.

Type in the score for each developmental area.
- Select the follow-up actions taken.

Click SAVE.
Note:
- DAISY provides an option to enter additional questionnaires for program sites who wish to do so. Adding additional questionnaires outside the three-time points is optional.
- The exemption status option is to accommodate data entry for parents who will not be asked to complete an ASQ-3 questionnaire because their child is already diagnosed with a disability or developmental delay. By using this option, users will identify families who are "exempt" to avoid the appearance of incomplete data entry. - If your site uses ASQ online to collect ASQ-3 data, ASQ-3 data entry is not required to be entered in DAISY. Instead, you will be asked to submit a CSV file of ASQ-3 data from ASQ online annually by January 31st.
