How do I add a Family Encounter Log? (Washington State sites only)

The Family Encounter Log is a required form for Washington State sites.

• Starting on the family record that you would like to add a new Family Encounter Log to, you will then navigate to the form section.
• Click ADD FORM
• Under Washington State Only, click Family Encounter Log from the pop-up window.

• Enter the date and type of encounter. You can also enter a description.

• Click SAVE.
