This guide walks you through the process of starting and completing your annual KEEP form. Follow each step in order, and remember that you can save your work at any time and return to it later.
- Open your site page
- Find your site name on the main screen.
- Click View next to the site name to open the site page.
2. Go to the KEEP form
- In the site menu, select Programs.
- Hover over Annual Requirements.
- Click Fillable Forms.
3. Add a new form
- Click the ADD FORM button.
4. Pick the correct year
- Select the year the KEEP is for.
- Example: For forms submitted in January 2026, choose the 2025 KEEP year.
5. Complete Section 1
- Review the information shown and make sure it is correct.
- If something is wrong, explain the issue clearly in the open text box.
6. Complete Sections 2-4
- Answer each item with Y (Yes), N (No), or P (In Progress).
- For open-ended questions, type your complete answer in the space provided.
- The KEEP form is long, so DAISY automatically saves your progress as you go.
If you need to stop before finishing
- Click the X in the top right corner of the form to exit.
- In the list of existing forms, your KEEP form will appear as Incomplete.
- To finish it later, click the Edit button next to the form.
