How do I add a PIEL measure?

The PIEL is a parent measure collected pre-program, mid-program, and post-program for each in the 1:1 model.  To add a PIEL measure, follow these steps:

• Click ADD FORM.
• Select PIEL from the pop-up window.
• Type in the administration date (the date which these data were collected).

• Select who was present during the assessment. This dropdown will include all adults listed in the Family Information form.

• Select the form type (pre, mid or post).

• Select the response for each question. Options range from rarely to always.

• Click SAVE.
