How do I add a FCCERS?

The FCCERS is collected pre-program and post-program in the HBCC model. To add a FCCERS measure, follow these steps:

• Click ADD FORM.
• Select FCCERS form from the pop-up window.

• Select the form type (pre or post).

• The FCCERS form on DAISY is designed to match the FCCERS score sheet. Copy the score for each item on the score sheet in the corresponding fields.

• Click SAVE.
